Regarding the Manual Yes/No task -- do I need to create the task, or is
it built-in?
Where is it added in a workflow?

You can add a Misc -> Manual Yes/No task to a workflow. This will prompt

a user role for confirmation, then fire off another workflow.

May I ask what Anthill version and browser you are using? I just tried 
this with 3.4.6 and Firefox and wasn't able to reproduce.
